Well Visits

Your baby's first visit with us is scheduled for 24-72 hours after discharge from the hospital, again at 2 weeks of age, 1 month of age, and then in accordance with the vaccine schedule listed below. Once your child is 3 years of age, well visits are scheduled annually. Intermittent appointments for weight checks may occur between well visits if needed.

At all well visits, growth and development is reviewed and anticipatory guidance is offered dependent on the age of your child. Please bring questions that relate to your child's growth and development to these visits.

We follow a vaccine schedule supported by the American Academy of Pediatrics. Vaccines and Your Baby Video Series

1 month: Immunizations given - Hepatitis B
2 months: Immunizations given - Pentacel (DTaP, HIB, Polio), Prevnar, Rotateq
4 months: Immunizations given - Pentacel (DTaP, HIB, Polio), Prevnar, Rotateq
6 months: Immunizations given - Pentacel (DTaP, HIB,Polio), Prevnar, Rotateq
9 months: Immunizations given - Hepatitis B
1 year: Immunizations given - Prevnar, MMR, HgB, Lead screen
15 months: Immunizations given - Varicella, Hepatitis A
18 months: Immunizations given - Pentacel(Dtap,Polio,Hib), Hepatitis B
2 years: Immunizations given - Hepatitis A
2 1/2 years: No vaccines (unless missing doses)
3 years: No vaccines (unless missing doses)
4 years: Immunizations given - MMR, Varicella
5 years: Immunizations given - DTaP, Polio
11-12 years: Immunizations given - Tdap, Menactra, Gardasil
16-17 years: Immunizations given - Menactra booster

At 5 years of age, your child will need to be scheduled for their "kindergarten physical". This appointment may be scheduled shortly before or shortly after their 5th birthday. This physical will include booster immunizations or DTaP. At this appointment you will be provided with HRS forms required for school admission: physical form (Gold) and shot record (Blue).

If your child is in day care, prior to starting school, you may need these forms sooner. Be sure to let us know you need these forms when you schedule your appointments.
